Southeast Regional League - US Club Soccer

GAME REPORT:

[Game Report Form]

The home team is to bring a blank copy of the Southeast Regional League Game Report to each game, at which time both teams will add their game day rosters to the Game Report.

The home team is responsible for returning the completed (including referee initials) Southeast Regional League Game Report to US Club Soccer within 24 hours of the conclusion of the match.

The form can be returned via e-mail to Operations Manager Daniel Allen at dallen@usclubsoccer.org (preferred) or via fax at (843) 614-4146.

GAME CHANGE REQUEST:

[Game Change Request Form]

All requests for schedule changes must be approved by US Club Soccer. Requests must be submitted at least 28 days in advance of the currently-scheduled game via the Game Change Request Form.

The form should be returned to Operations Manager Daniel Allen at dallen@usclubsoccer.org.
